Submitting Incident Forms

Select one of the Official Scouting America fillable PDF documents and submit it within 24 hours.

Before choosing one of the three methods below to report your incident to Scouting America, ask yourself a question: Is the incident I am reporting a Youth Protection incident?

If YES:

  • Contact the Department of Families and Children (DCF) to report a possible child abuse or neglect by following the instructions here.
  • After reporting the possible child abuse or neglect to DCF, please request an incident case # to provide with the report to Scouting America.
  • Once this you have obtained the incident case #, please chose one of the three below methods to report the incident to Scouting America.

If NO: 

  • Please chose one of the three below methods to report the incident to Scouting America.

Following the completion of the Incident reporting, please ensure the claimant is provided the Health Insurance Claim Form available here.

(Accident and sickness insurance coverage for Scouts and Scouters furnishes medical reimbursement within the policy amounts of death, accident, or sickness. These plans and policies are excess of all other available sources of medical insurance or other healthcare benefits. If there is no additional primary insurance or healthcare plan, these policies generally pay as primary coverage, subject to the plan limits and terms.)

General Incident Report

Form 680-016

Use this tool to report injuries, illnesses, and incidents requiring medical attention beyond basic first aid. Suitable for situations occurring during Scouting activities or on council-owned properties.

Near-Miss Report

Form 680-017

Use this to report near misses – incidents that could have resulted in harm but did not. Valuable for identifying safety and program practice improvements.

Youth Protection Form

Form 680-67676

For reporting instances of abuse, violations of Scouting America policies, or inappropriate behavior involving Scouts, Scout leaders, parents, or others.

Submission

Verify that all fields are complete. If you are reporting a youth-related incident, ensure the privacy of all involved is maintained as per Scouting America guidelines.

Messenger of Safety

Becoming a "Messenger of Safety" is more than just a patch; it's a commitment to bringing a culture of awareness and safety to your unit.

In Scouting, we do not comprise the safety of our youth, volunteers and employees. Safety is a value that must be taught and reinforced at every opportunity. We are all responsible and must hold each other accountable to provide a safe environment for all participants.
